Locating your service area
The Health Plan Medicare Part D Prescription Drug Formulary is a listing of covered prescription medications. Prescriptions must be filled at participating network pharmacies in most cases. Please see the formulary documents at the links, below.
What if my drug is not included in the plan formulary?
If your drug is not included in the plan formulary:
- You can contact Customer Service to confirm that a drug is not covered and ask how to make an exception and cover your drug.
- You can reference the section in the formulary, "What if my drug is not on the formulary?" for the options available to you.

Changes to the formulary
The Health Plan may add or remove drugs from the formulary during the year. To get up to date information about covered drugs, please access the medication search feature through Express Scripts at the link(s) below. You can also contact us at 1.800.592.4465 (TTY 711), 24 hours a day, seven days a week, except Thanksgiving and Christmas.
By selecting one of these links, you will leave The Health Plan Medicare Advantage website and access the Express Scripts, Prescription Drug Coverage search page. Choose the plan and click the link for up to date medication information.”
